When leaving home involves considerable effort, complex support or mobility difficulties, it may be appropriate to ask about a geriatric home visit. Meeting in familiar surroundings can help in understanding everyday function and discussing needs with the patient and those supporting them.

Home visits from Medical Experts are arranged in advance with the team. Before confirming a visit, we clarify the reason for the request, the patient’s address and the type of assessment required. The option of a home visit does not mean the service is available in every locality or for every medical need.

A home visit may suit someone with restricted mobility, weakness after a hospital stay, substantial assistance needs or another difficulty reaching the clinic. Suitability depends on the situation and whether the required assessment can be carried out at home.

The visit may consider health, medication, memory and the help needed for everyday activities. Observing the home environment and support available can add information that may be less apparent in a clinic visit.

A planned home visit is not an emergency service and cannot replace every medical investigation. Blood tests, imaging or further assessment elsewhere may be needed. A sudden change, such as new confusion or significant breathlessness, requires urgent medical attention rather than waiting for a visit.

What to expect at the visit

We begin by getting to know the patient and discussing their health and main questions. The doctor reviews records and medication and asks about the daily routine at home.

Medical and functional assessment is tailored to the need and conditions. It may address mobility, memory, mood, nutrition and the support the patient receives. With their consent, a relative or regular caregiver can participate.

Towards the end, the findings and recommended next steps are discussed. Clarify which investigations are needed, how information will reach the treating doctor and how to arrange follow-up. The scope of the visit and any particular document required should be discussed beforehand.

How to prepare

When arranging a visit, provide the locality and address, access details, reason for the request and a contact person. The team will check suitability and availability at that address.

Have a medication and dosage list, hospital summaries, test results and relevant records ready. Write down questions and examples of recent changes in daily function.

Prepare a quiet, comfortable place for the discussion and examination. If the patient agrees, having someone familiar with the routine present can help provide information and understand recommendations. Appointment and service-area enquiries: 052-351-0304.

Frequently asked questions

Are visits available in every locality?

Provide the address and check availability with the clinic team. The service area and ability to attend are confirmed in advance for each request.

Can a home visit include memory assessment?

Part of an assessment can sometimes take place at home. Suitability and scope depend on the need, and additional investigations elsewhere may be required.

Is a home visit an alternative to hospital or emergency care?

No. This is a planned assessment and consultation. An emergency or sharp deterioration requires appropriate medical attention without waiting.

What should be confirmed before arranging the visit?

Ask about the cost, duration, planned assessment, required documents and follow-up. Mention beforehand if a medical opinion for a particular purpose is needed.
