Geriatric assessment connects the medical picture with life at home: what has changed, what troubles you and what matters to you to maintain.
At Medical Experts in Kiryat Motzkin, consultations are provided by Dr. Maher Hijazi, a specialist in internal medicine and geriatrics. The assessment aims to understand needs, identify priorities and develop recommendations suited to the person's condition and preferences.
Who might benefit? Older adults with several chronic conditions, multiple medications, repeated falls, mobility difficulties or changes in independence. An assessment may also be helpful after a hospital stay or when relatives notice a persistent change and want to understand how to help. Someone who remains independent may also need assessment for a new difficulty.
Depending on the need, the assessment considers medical conditions, medication, memory and thinking, mood, nutrition, pain and everyday function. There is no identical set of tests for everyone: the scope depends on the discussion, findings and reason for seeking advice.
The consultation complements care from the family doctor and other treating clinicians. It does not guarantee a particular improvement or replace urgent care. Sudden confusion or a sharp deterioration requires urgent medical attention rather than waiting for a planned appointment.
We begin by discussing the reason for the visit, medical background and daily life at home. You can describe activities that have become harder, a change after hospital treatment or questions about existing care. With the patient’s consent, a relative who knows their situation can take part.
Medical records and medication are reviewed, and medical and functional assessments are tailored to the need. The assessment may also consider how memory, sleep, mood or nutrition affect everyday life.
The findings and next steps are then discussed. Recommendations may include further investigations, reviewing medication, changes to daily routines or referral to other professionals. Clarify what needs to be done, who will coordinate ongoing care and when follow-up is needed. Medication changes should only be made under medical guidance.
Prepare an up-to-date list of all medications, doses and supplements, together with relevant hospital summaries, recent tests and previous medical opinions.
Write down two or three main questions and examples of changes in daily function: when they began, when they are noticeable and what help is currently needed. Bring glasses and hearing aids if used.
If the patient wishes, come with someone familiar with their everyday life. Before the appointment, ask the team about the visit length, cost and required documents. Appointment enquiries: 052-351-0304.
Not necessarily. Persistent changes, several medical problems or difficulties in everyday life can be discussed with the treating doctor to consider whether an assessment is appropriate.
No. It complements ongoing treatment and can help build a broader picture and guide follow-up.
Medication review is an important part of assessment. Bring a complete list, including over-the-counter medicines and supplements. Do not stop or change treatment yourself.
You can ask whether a home visit is suitable. This depends on medical needs, the address and availability, and must be arranged with the clinic in advance.
